Home School Partnership

For students to be granted admission to the school the following forms are required:

CHECKLIST FOR ENROLLMENT

Registration Fee (Non-Refundable)

Testing Fee (Non-Refundable) when needed

Completed and signed Registration form

Completed Enrollment form

Signed Consent of Parents to Release Student Records

Signed Parent “Doctrinal Statement”

Signed “Statement of Agreement”

Completed Medical and Emergency contact and authorizations (current)

Completed Activity Selection form

Transcript (Grades, Achievement Test Scores and any special testing or special ed. records)

Copy of birth certificate

Immunization records (current)

Signed TCPS AV release

Signed TCPS Acceptable Computer Use form

Completed Kindergarten Questionnaire

Completed Student Questionnaire (7-12)

Completed Student Autobiography Essay (9-12)

Signed Student Conduct Agreement and Faith Statement (7-12)

Completed Student Essay (7-12)

Completed Administrator Recommendation form (1-12)

Completed Teacher Recommendation form (1-12)

Completed Minister Recommendation form

Completed TCPS admission test results

Interview with an administrator

Payment of other fees and tuition
